Record Management	How do we know you?
	We desire to fulfill the worship and spiritual needs of the body of our church. An Informational Profile Form is a great way for us to get to know you and your family. It gives the church information about an individual or family that can be used to track church growth and ministry opportunities as well as providing current information for the church directory, newsletter, bulletin and other special mailings. Everyone who attends our church is encouraged to complete a form and to keep your information up-dated as it changes. You can download a form through the link below or pick one up in the Church Clerk’s office. Also, you can e-mail your changes through the link below. Thanks so much for your help. If you are needing to up-date your existing information, just enter the information that needs to be changed or added. · If at anytime you would like to complete a form, blank forms are available on the church website, or in the Church Clerks Office. Completed forms can be turned in to an Usher, a Greeter or to the Church Office. · Please list all children living at home along with their birth dates. This can provide a rich history of information as you and your children grow in the future of the church, as well as when they begin to have families of their own. · Proper record keeping is imperative to our continued church growth and ministy potential.
